2.1 on 1.7 site, ADD option doesn't work

clean install of xmap 2.1 on Joomla 1.7 site.
When I try to ADD sitemap, I get the screen in the attachment.
It has a title that says "site details", has an entry field for Title, Alias,Status, and Access, but no way to save the data. It is like there is no template for the form.

Hi,
I haven't seen that error before! is it possible that you can see your PHP error log messages to see if anything shows up there?
