Trouble Generating Multi-Level Sitemap

I'm sure there is something simple I am missing here, but I am unable to generate a sitemap that includes anything other than the top level menu items.

For example, if a menu contains category A,B,C and there are dozens of pages in each category, my sitemap only generates 3 entries - A,B,C.

This goes both for content as well as my VM and SMF pages.

Any tips on this? My sitemap is no more useful than my menu. Tried on multiple sites with various plugins and my sitemap simply mirrors whatever menu I add to it.
