Can't get Extra Vote to work.

Hello,
I installed Extra Vote on my test website http://bigpete.cloudaccess.net/ , After several attempts I can't see it anywhere. I use DJ Catalog 2 for my web store (http://www.bigpetestf.com/our-fish/item/1-african-cichlids/1-electric-blue-johannii-melanochromis-johannii.html) and I would love to incorporate Extra Vote for product ranking.
